Treatment of multi-resistant necrotizing fasciitis infection in dog tail

Three year old English Mastiff develops an infection in the tail. The analysis shows that it is caused by a multi-resistant Staphylococcus pseudintermedius – necrotizing fasciitis, also commonly called flesh-eating bacteria. Amputation is recommended, but the owner wants to try to save his dog’s tail.

SertaSil is sent by courier to Texas, US and treatment started immediately. It consists of very thorough daily washing with tap-water followed by gently drying and the application of a fully covering layer of SertaSil. The wound is not covered by a dressing. SertaSil is used once daily for 4 days after which only tap-water and access of air is used, except for a single application Day 34.

No complications occurred during the treatment. The wound closed fully and has remained closed.

Wound Day -2 before start of SertaSil. Infection is rapidly spreading and the tissue is dying.
Morning of the start of SertaSil. Dead tissue has mostly been removed, but the infection is continuing to spread.
Day 0 immediately before start of SertaSil. The infection is rapidly spreading in the tissue and is causing tissue death. The redness is extending towards the rear part of the body, threatening to affect the torso itself.
Day 0 - first application of SertaSil.
Day 1 - 24 hours after first SertaSil application and after washing with tap-water. Clear and rapid improvement.
Day 4 - after daily application for 4 days. This is the last day of SertaSil application.
Day 5 - after 4 days of SertaSil application and 1 day of only washing with tap-water.
Day 8 - After the initial 4 consecutive days with SertaSil treatment, the wound was only washed thoroughly with running tap water daily and left uncovered at all times. No antibiotics, antiseptics or other treatments or dressings were used at any point.

Day 14 - The new skin has natural pigmentation and hair follicles – not typical of altered scar tissue.
Day 17 - Scab is correctly left in place along the cranial wound edge.
Day 18 - Light coloured dots and patches in wound bed are new skin.
Day 23 - The new epithelium in wound bed was unfortunately scrubbed off during washing. Treatment after Day 4 still only consists of washing with tap-water and access to air.
Day 29 - The scab formation along the wound edges is correctly left intact during washing.
Day 33 - An excessively harsh cleaning has removed the healthy scab along the wound edges.
Day 34.
Day 36 - Following the initial 4 consecutive days of SertaSil treatment, the wound had only been washed thoroughly using running tap water daily and left uncovered at all times. On Day 35, the opportunity of using SertaSil again led to its daily use again going forward. No antibiotics, antiseptics or other treatments or dressings were used at any point. During the last 24 hours with SertaSil considerable scab formation is seen along wound edges
Day 49 - Healthy skin with natural pigmentation, hair follicles elasticity and protection has regenerated. - not fragile scar tissue. Healthy scab correctly left in place. It will fall off by itself as the skin underneath matures.
Day 63 - Final scab has fallen off. The wound is closed. Regeneration will continue over the coming months to years.
Day 77 - The scar is transforming into genuine skin with hair follicles, glands and pigmentation.
